Neonatorum between the Group given Prednisolone with the Conventional Therapy and the Conventional Therapy Alone

Abstract
Tetanus neonatorum is a disease that is found in a considerable number of newborn infants in this country because many of them treat the umbilical cord with unsterilized instruments in their home deli-very without help of doctors and midwives and the mortality rate is still high with the conventional therapy alone.
This report is concerned with 21 cases that we have seen in the Severance Hospital during the period from October, 1959 to August, 1969 They were devided into two groups for comparative observation between the conventional therapy (C.T.) and the C.T. with steroid therapy. The C.T. given to all patients include the tetanus antitoxin, penicillin, sedatives and oxygen inhalation p.r.n. in a dark room. One third of the cases were also treated with prednisolone 10mg./day through gastric tube in addition to the above C.T.
It is considered that the administration of prednisolone is unusally correlated with a drop in temperature and fall in the pulse rate as well as a diminution in the intensity and severity of the convulsion and trissmus from this observation. Fever often, recurred when the dose of prednisolone was reduced to 5mg./day in some cases. Of the 14 controls, there were only two patients survived, whereas 5 of 7 patients getting
steroid horrnon with t le C.T. were recovered. Beside the above findings even the two dead cases sur%7ived
longer than the group given the C.T. alone.
Since the authors did not find any harmful effect of steroid therapy on the observed cases as long as the patients were given the other conventional therapy, they recommend to use steroid hormone adequately with the conventional therapy when we meet neonatal tetanus to save more patients.
